What makes a persistent message get backed out to a back-out queue? Why should that circumstance be any different for a non-persistent message?

MQ will transactionally backout (rollback) a message to the original queue if it was got under syncpoint and the application failed.

If you mean moved to a backout queue, yes that is application dependent.

Well, I don't think there is any question about it. It can only be attributable to human error. This sort of thing has cropped up before, and it has always been due to human error.
