what may be the reason for the following failure?

Check the log file for the installation command for an explanation.
Example: /users/root/smit.log or /var/adm/sw/swagent.log etc....

Well it could be just about anything. Personally my money goes on it being a user error.

Call it a hunch, call it a SWAG...... but that is my bet.
