In the MQ documentation it is explicitly called out that Multi-instance queue managers dont work with Oracle ACFS files system.

But if I am NOT using multi-instance, then will MQ work without any issue on ACFS file system ?

AFAIK, Queue Managers that are not using MIQM do not need to meet the requirements on the MIQM Testing Statement web page.

states for non-MQIM :
"Today, networked file systems can provide good performance, and implementations of reliable network file system protocols such as RFC 3530, Network File System (NFS) version 4 protocol, meet the requirements for logging messages reliably."

The issue is fixed when we moved the Queue manager log/data files to a local storage. Looks like it is an issue in the network - the previous ACFS files system had to be mounted from another network, and required multiple n/w hops.
Looks like when there is an IO operation delay, client connections can go up and can cause errors.
