Websphere Data Interchange integrates nicely with WMQI for handling the parsing, mapping, etc.

Since HIPAA is health-inductry specific, you may find yourself dealing with HL7 Messages, in which case you may need an HL7 Parser...see the IA0B Support Pack at http://www-3.ibm.com/software/integration/support/supportpacs/product.html#wmqi but be warned, this may require some rework depending on your companies HL7 messages.
