We have a nice new MSCS based Windows 2003 server Queue Manager with four CPUs (dedicated to MQ 5.3 usage).

To get most out of such a platform with multiple CPUs - is it necessary to have more than one queue manager installed (as I understand that the MQ code is single threaded)?

WMQ code is not single threaded. There is a single thread which handles connection requests, but after that the agent processes are multi-threaded by default.
WMQ is much more likely to be IO bound rather than CPU bound. The speed that data can be written to the logs and queue files is much more of a factor than the speed of the CPU.

Thanks, unfortunately our SAN has a single RAID-5 array of 7+1 disks for both the MQ queues and logs. I am trying to get separate disks allocated for the log files to improve the QM performance.

Don't forget to increase your licenses if you every have the chance of being audited. MQ is by the processor.
