Is anyone using email to notify a person on first or second notification instead of placing the notification on a worklist? Would it be easier to expire the workitem to trigger the email instead of using the notification feature?

This is done often... It is your choice as to how you implement.
If you use Notification, you will need some type of utility to run on a cron, that monitors for the Notifications and sends the emails.
If you use Expiration, you will need to send the email, then loop around back to the activity a second time, possibly making sure you do not expire it again.

Where/what can you monitor for these notifications. I need to send out email when notification happens. I've got skill available to write something in C or Java, but I do not know what to watch for. Any pointers please.

You have 2 choices:
1. Write a program that you run on a cron that periodically scans for Items of TYPE Notification then process those notifications.
2. Use the Audit to MQ facility and watch for the Notification event.
