How do we implement active-active failover

It depends on the platform.

AIX has HACMP, HP has ServiceGuard or Multi-Compter/ServiceGuard, Compaq has TruCluster, Solaris has SunCluster or Veritas, and Windows has Clustering.

Take a look at the relevent Support Pacs.
