Guys, I was having the same problem with the queue manager not being able to start. Here is the skinny:
Setup:
workflow server1 on server X
workflow server2 on server Y
Oracle 9.2 database on server Z (this one serves both X and Y workflow servers)
problem:
Workflow configuration on server X (first queue manager in the cluster FMCQM) went ok.
Workflow configuration on server Y (Additional queue manager in the cluster FMCQM2) gave me the following:
/home/fmc #> strmqm FMCQM2
AMQ7626: XA resource manager initialization failure. Refer to the error log for
more information.
Question:
I found that by answering "NO" to the "do you want to create the queue manager FMCQM2 now" question, and creating the queue manager later it actually starts ok. Is this advisable? can we create the queue manager manually instead of using the configuration?
any help will be greatly appreciate it.

Guys, that didn't work. Here is my error log file:
var/mqm/qmgrs/FMCQM2/errors #> cat AMQERR01.LOG
11/18/05 15:22:34
AMQ6175: The system could not dynamically load the library
/usr/lpp/fmc/oraswit/oraswit. The error message was Could not load module
/usr/lpp/fmc/oraswit/oraswit.
Dependent module libclntsh.a(shr.o) could not be
loaded.
Could not load module libclntsh.a(shr.o).
System error: No such file or
directory
Could not load module /usr/lpp/fmc/oraswit/oraswit.
Dependent module
/usr/lpp/fmc/oraswit/oraswit could not be loaded.. The queue manager will
continue without this module.
EXPLANATION:
This message applies to UNIX systems. The dynamically loadable file
/usr/lpp/fmc/oraswit/oraswit failed to load correctly due to an internal error.
and the MQ error recording routine has been called.
ACTION:
Check that the file has not been corrupted then use the standard facilities
supplied with your system to record the problem identifier, and to save the
generated output files. Contact your IBM support center. Do not discard these
files until the problem has been resolved.

AMQ7622: WebSphere MQ could not load the XA switch load file for resource
manager 'wflprd01'.
EXPLANATION:
An error has occurred loading XA switch file /usr/lpp/fmc/oraswit/oraswit. If
the error occurred during startup then the queue manager will terminate. At all
other times the queue manager will continue without this resource manager
meaning that it will no longer be able to participate in global transactions.
The queue manager will also retry the load of the switch file at regular
intervals so that the resource manager will be able to participate again should
the load problem be resolved.
ACTION:
Look for a previous message outlining the reason for the load failure. Message
AMQ6175 is issued if the load failed because of a system error. If this is the
case then follow the guidance given in message AMQ6175 to resolve the problem.
In the absence of prior messages or FFST information related to this problem
check that the name of the switch load file is correct and that it is present
in a directory from which it can be dynamically loaded by the queue manager.
The easiest method of doing this is to define the switch load file as a
fully-qualified name. Note that if the queue manager is still running it will
need to be restarted in order that any changes made to its configuration data
can be picked up.

I think its an unix linking issue. Did you try settng the proper links on the server2 box for the user id which installed workflow ( typically fmc )? _________________ Thanks

Koko,
Yes, indeed. That was the issue and it has been fixed already. Thank you very much for your help.
