Well, I don't think there is any question about it. It can only be attributable to human error. This sort of thing has cropped up before, and it has always been due to human error.

It's very important both for your long term career and for the respect you get at your current job that you learn how to learn new technology.

This includes being able to find information on your own, and being able to read documentation and follow it. It includes understanding how to configure small test environments so you can prove both your understanding and your solution without causing impact on environments being used by other people.

It's a very necessary skill for any job involving hands on work with technology - if not the most important skill you can have.
